Arizona Care Network (ACN) is a clinically integrated (CI) physician network and Accountable Care Organization (ACO) sponsored by Dignity Health and Abrazo Health.  The Field Manager Embedded Care is an employee of MedProVidex, a wholly-owned subsidiary of Dignity Health. 

 

Under the direction of the Director of Embedded Care, the Field Manager Embedded Care is external facing and manages Embedded RN Specialists and Social Workers who are either in the field or centrally embedded.

 

The Field Manager Embedded Care will oversee the embedded services for ACN including maintaining relationships with the vendors of embedded services, as well as providing management for RN Care Specialists and Social Workers. 

 

 
PRINCIPLES DUTIES AND RESPONSIBILITIES

  • Oversee the daily, onsite, management activities of the Embedded Care RN Specialists and Social Workers to include:

o             Onsite and home visits for the complex patient, staff mentoring, coaching, supporting, and counseling.

o             Development and implementation of performance improvement plans.

o             Daily staffing and census management based on Population Health and Risk Stratification, TEAM timecard oversight.

o             Assist in complex patient/family management issues.

o             Perform documentation audits per department guidelines.

  • Coordinate with ACN Transitional Care Team based on risk stratification and established care plans.
  • Assist practices with daily workflows as identified by their Population Health needs.
  • Responsible to assist Embedded Care Director with hiring process of Embedded Care Social Workers and Embedded RN Specialists.
  • Implement Population health strategies at the practice level.
  • May participate in direct care coordination activities as deemed necessary.
  • Assists in orienting new care coordination staff as necessary.
  • Possible backup to the Manager Embedded Care Navigators when necessary. 
  • Performs other duties as assigned by Management.

 
POSITION QUALIFICATIONS

A)           EXPERIENCE:  Minimum 3 years’ experience in clinical operations including post-acute care, transitions of care-related work, and throughput experience in a provider setting, some of which is at the management level. Knowledge of community resources.   

B)            EDUCATION AND TRAINING:  Bachelor of Nursing or equivalent experience required. Master’s Degree in related field preferred.

C)            LICENSURE:        Registered Nurse in Arizona.

D)           SPECIALIZED KNOWLEDGE AND SKILLS:  Demonstrated understanding of post-acute care operations and strategic positioning of post-acute programs and services.  Able to articulate an understanding of the continuum of care, including acute, post-acute, and community-based services.
